“Fraud and waste” are terms often used in government and business to describe different types of misconduct. These are not interchangeable terms, yet they do go hand in hand! Fraud refers to intentional deception for unfair or illegal gain, while waste can refer to the mismanagement or improper use of human resources or, in some contexts, to literal human excrement – the kind of waste Jasmine Hoyte used to commit a multi-million dollar Medicaid scheme.
Hoyle owned and operated two companies that offered substance abuse treatment services, Harvest Focused & Consulting Services LLC and The Ultimate Sacrifice. Both businesses billed Medicaid for more than $6.7 million in services that were never performed, including significant quantities of urine drug tests. And in many cases, there were multiple claims for the same Medicaid beneficiaries. For instance, from August 2020 through July 22, Hoyle submitted claims for 97 drug tests to Medicaid on behalf of a person who had never heard of either business or had provided any urine for the drug tests.
Harvest Focused & Consulting Services LLC and The Ultimate Sacrifice offices actually had no laboratory equipment that could be used to run urine drug tests. When asked about the urine drug testing program, Hoyle admitted that some of the urine that was used for the urine drug screens were provided by a friend, family members of the friend or members of a motorcycle club. Turns out only about 10% of the billing for Harvest Focused and The Ultimate Sacrifice was legitimate, meaning the rest really was just a waste.
On April 9, 2025, Hoyle pleaded guilty to healthcare fraud.

According to the Middle District of North Carolina, Jasmine Hoyle, 35, pleaded guilty to one count of health care fraud and one count of money laundering in connection to a scheme to defraud North Carolina’s Medicaid program.
Hoyle owned and operated two Winston-Salem-based businesses, Harvest Focused & Consulting Services and The Ultimate Sacrifice. Both of these businesses billed Medicaid for “significant quantities” of drug tests. Hoyle submitted claims for 97 drug tests to Medicaid between August 2020 and July 2022 on behalf of a person who had never heard of either business and had never provided any urine for the drug tests.
